Instead of just being able to send just a test from the app I'd like to be able to send a custom notification from the app.

As a user, I disagree. If you want to send notification by app, it should be a new app. In general, we don't receive the notification alone, we broadcast to many people. If we allow people can send notification back and forth as well, it you end up with messy.

I am unsure what you mean by custom notification.
You can mark any link, image, text then click share and share via ntfy app to any topic?
What more could you want?
